Job summary

Matrix Medical Network is seeking a licensed Nurse Practitioner to conduct in-home Comprehensive Health Assessments (CHA) for adults, including geriatrics. You will review medical history, medications, and social factors to identify health risks and close care gaps.

The role involves coordination with physicians, case managers, and family members, performing point-of-care testing, and visiting patients at their homes or Skilled Nursing Facilities as needed.

Qualifications

• Master's NP degree and current NP licensure in applicable state.
• Board certification in Adult/Family/AGNP/ACNP/EM/Care specialties.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads may apply.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, treatment, and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers and families as appropriate.
• Provide services at members' residences (home or SNF).
• Document findings clearly following Matrix guidelines.
