Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform in-home Comprehensive Health Assessments. You will review medical history, medications, and social factors, and document findings to close care gaps. Collaborative work with physicians and care teams ensures quality outcomes.

The role offers flexible scheduling, across the nation, with ongoing education and opportunities for licensure support and credentialing as needed.

Qualifications

• Master’s degree in nursing (NP) required.
• Active NP licensure in practice state with prescriptive authority.
• AANP/AACN/ANCC board certification in approved NP specialty.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance.
• Proficiency with clinic software and health records systems.

Responsibilities

• Perform adult/geriatric health assessments in patient homes.
• Review medical history, medications, and social factors; document findings.
• Coordinate care with PCPs, case managers and other clinicians.
