Job summary

Herewith is seeking compassionate Care Partners to support dialysis patients in-home in Walnut Creek, CA. You will help prep treatment, ensure safety, and monitor wellbeing.

You will be matched to patients based on skills and personality and supported by a clinical team.

Key duties include setting up machines (NxStage, Tablo), providing companionship, monitoring safety, and documenting concerns.

Qualifications

• Experience supporting dialysis patients in home or clinical settings.
• Familiarity with home dialysis machines (preferred).
• Reliable, empathetic and detail-oriented.
• Strong interpersonal communication.
• Must pass background check and vetting via Herewith platform.

Responsibilities

• Conduct and support patient setup for dialysis treatments using machines such as NxStage or Tablo.
• Provide companionship, emotional reassurance, and schedule adherence.
• Monitor safety and assist with light activities of daily living (ADLs).
• Maintain communication with clinic nurses and care coordinators.
• Document any concerns or changes observed during support hours.
