Job summary

Herewith is seeking compassionate and reliable Care Partners to support patients receiving dialysis in the comfort of their own homes in Bodfish, CA. You will have at least 1 year of dialysis machine experience and will prep and monitor treatments while ensuring a safe home environment and patient wellbeing.

You will collaborate with clinic nurses and care coordinators, document changes, and provide emotional support.

Qualifications

• Prior experience supporting dialysis patients (home or clinical setting).
• Familiarity with home dialysis machines (e.g., NxStage).
• Reliable, empathetic, and detail-oriented."
• Strong interpersonal communication skills.

Responsibilities

• Set up and support patient dialysis treatments.
• Provide companionship and emotional reassurance; assist with scheduling adherence.
• Monitor safety and assist with light ADLs in the home.
• Maintain communication with clinic nurses and care coordinators.
• Document concerns or changes observed during shifts.
