Job summary

Herewith is seeking compassionate In-Home Dialysis Care Partners to support patients receiving dialysis at home. You will ensure patient safety, assist with treatment prep, and monitor wellbeing, working closely with a clinical team.

Ideal candidates have at least 1 year of dialysis experience, strong reliability, and excellent communication. The role offers consistent weekly hours and a supportive care network to ensure patient comfort at home.

Qualifications

• At least 1 year of dialysis machine experience.
• Reliable, empathetic, and detail-oriented.
• Must pass background check and vetting via Herewith platform.

Responsibilities

• Conduct & Support patient setup for dialysis treatments (e.g., NxStage, Tablo or equivalent machines)
• Provide companionship, emotional reassurance, and schedule adherence
• Monitor safety and assist with light activities of daily living (ADLs)
• Maintain communication with clinic nurses and care coordinators
• Document any concerns or changes observed during support hours
