Job summary

Herewith is seeking an In-Home Dialysis Care Partner to support patients receiving dialysis in their homes. The role involves setting up treatments, ensuring safety, and providing compassionate care under clinical supervision.

Candidates should have at least 1 year of dialysis machine experience, be reliable and empathetic, and able to communicate well with patients and care teams. Training and placement assistance are provided.

Qualifications

• Prior experience supporting dialysis patients (home or clinical setting).
• Preferred: Familiarity with home dialysis machines.
• Reliable, empathetic, and detail-oriented.
• Strong interpersonal communication.
• Must be dependable.
• Must pass background check and vetting via Herewith platform.

Responsibilities

• Conduct & Support patient setup for dialysis treatments (e.g., NxStage, Tablo or equivalent machines).
• Provide companionship, emotional reassurance, and schedule adherence.
• Monitor safety and assist with light activities of daily living (ADLs).
• Maintain communication with clinic nurses and care coordinators.
• Document any concerns or changes observed during support hours.
