Herewith is seeking an In-Home Dialysis Care Partner to support patients receiving home dialysis in Thousand Oaks, CA. You will ensure a safe environment, assist with treatment prep, and monitor wellbeing under a clinical team’s guidance.

The role emphasizes compassionate care, independent work with clinical support, and consistent scheduling. Prior dialysis experience and a background check are required.

Qualifications

• At least 1 year of dialysis machine experience.
• Prior experience supporting dialysis patients.
• Must pass background check.
• Reliable, empathetic, and detail-oriented.
• Strong interpersonal communication.

Responsibilities

• Set up dialysis equipment for treatments and ensure patient safety.
• Provide companionship and schedule adherence during care.
• Monitor patient wellbeing and assist with light ADLs.
• Maintain communication with clinic nurses and care coordinators.
• Document concerns or changes observed during support hours.
