Job summary

Herewith is seeking an In-Home Dialysis Care Partner to support patients in Carrollton, TX. You will assist with setting up dialysis equipment, provide companionship, monitor safety, and collaborate with a clinical team to ensure patient wellbeing.

The role emphasizes independent, compassionate support within a home care setting, with consistent hourly scheduling and team backing for clinical needs.

Qualifications

• Prior experience supporting dialysis patients (home or clinical setting).
• Familiarity with home dialysis machines.
• Reliable, empathetic, and detail-oriented.
• Strong interpersonal communication.
• Must be dependable.
• Must pass background check and vetting via Herewith platform.

Responsibilities

• Conduct & Support patient setup for dialysis treatments (e.g., NxStage, Tablo or equivalent machines)
• Provide companionship, emotional reassurance, and schedule adherence
• Monitor safety and assist with light activities of daily living (ADLs)
• Maintain communication with clinic nurses and care coordinators
• Document any concerns or changes observed during support hours
