Job summary

La Frontera Arizona, through EMPACT-SPC, is seeking High Needs Case Managers to support families with high service needs in Tempe or Glendale. The role involves direct crisis intervention, transportation, and education to children and families, managing a caseload of 15 families with high-intensity service provision.

Requirements include a BHT-appropriate degree or higher with behavioral health experience, bilingual Spanish preferred, valid AZ driver license, fingerprint clearance, and reliable

Qualifications

• Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Bi-lingual (Spanish) preferred.
• Fingerprint clearance card or eligibility to obtain one is required.

Responsibilities

• Provide direct in-home, in-office, in-school, and community services including crisis intervention and transportation.
• Coordinate and document team meetings (ACT or CFT) and engage natural supports.
• Maintain 40 hours of work per week with 30 hours of direct client contact and timely documentation.
• Develop and maintain effective relationships with state, community, and contracting organizations.
• Ensure continuity of care and coordinate with Clinical Coordinator/Director as needed.
