Job summary

La Frontera Arizona's EMPACT program in Maricopa, AZ seeks a Children's Case Manager to provide direct crisis intervention, client transportation, and education to families and children. The role emphasizes collaboration with in-home services and community partners, with flexible scheduling to meet client needs. The ideal candidate has a Behavioral Health background and is 21+.

Bilingual Spanish is preferred. A valid AZ driver license and fingerprint clearance are required.

Qualifications

Bachelor's Degree in Behavioral Health or related experience preferred.
Experience with the Child and Family Team process.
Fingerprint clearance card or eligibility to obtain one.
Valid AZ driver's license with clean driving record.
Bi-lingual (Spanish) preferred.
Must be at least 21 years old.
High school diploma or equivalent for all employees.

Responsibilities

Provide direct service crisis intervention, transportation and education to families and children.
Complete family needs assessments and referrals to collaborating partners.
Coordinate communications with other departments and community partners.
Maintain professional conduct and positive working relationships with stakeholders.
Ensure continuity of care in provision of services.
