Job summary

Adobe Population Health is seeking a Part-Time Community Care Coordinator in Eugene, OR. The role combines field-based care coordination with in-home support to improve client outcomes across behavioral, physical, and social interventions.

Travel to Eugene and Lane County is required to provide in-home support and coordinate with providers, social services, and community resources. Prior care coordination experience and HIPAA compliance are essential.

Qualifications

• Four (4) years of related experience and/or training or equivalent.
• Medical experience.
• Experience with diverse populations, including underserved communities.
• Willingness to drive with a clean driving record.

Responsibilities

• Provide field-based care coordination to address behavioral, physical, and social needs of assigned clients.
• Assess social, financial, and health needs of individuals and families in the community.
• Connect clients with housing, food, healthcare, transportation, and mental health resources.
• Develop and implement individualized care plans with clients.
• Provide ongoing support, follow-ups, and reassessments.
• Advocate for clients to ensure timely access to services.
