Job summary

Adobe Population Health is seeking a Community Care Coordinator to provide field-based care coordination and management across Parker and surrounding areas. The role combines in-home support with supervisory elements to improve client outcomes through behavioral, physical, and social interventions.

The ideal candidate has 4–6 years of related experience, strong communication skills, and a willingness to drive.

Qualifications

• Four to six years of related experience and/or training.
• Medical experience strongly preferred.
• Experience with diverse populations, including underserved communities.
• Willingness to drive with a clean driving record.
• Reliable transportation.
• Excellent communication skills, both verbal and written.
• Ability to work in a self-directed environment.
• Knowledge of HIPAA compliance.

Responsibilities

• Provide field-based care coordination to address behavioral, physical, and social needs of assigned clients; reduce costs and improve quality of life.
• Assess social, financial, and health needs of individuals and families in the community.
• Connect clients with housing, food security, healthcare, transportation, and mental health resources.
• Develop and implement individualized care plans with clients.
• Provide ongoing follow-ups and reassessments to ensure needs are met.
• Advocate for clients to access timely services and resources.
