Job summary

Umpqua Health is seeking a licensed Care Coordinator to provide comprehensive care management for Medicaid/Medicare members across Douglas County, Oregon. The role involves in-home assessments, care plan development, and coordination with an interdisciplinary team to ensure high-quality, integrated care.

The candidate must hold an active Oregon RN license (BSN/MSN) with CCM eligibility within 18 months of hire, have 2+ years in healthcare (LTSS and care management), and be proficient with EHR

Qualifications

• Active, unrestricted Oregon RN license (BSN/MSN) with CCM eligibility within 18 months of hire.
• Minimum 2 years healthcare experience, LTSS and care management exposure.
• Valid driver's license and reliable transportation.
• Proficiency with Microsoft Office and EHR navigation.

Responsibilities

• Perform in-home assessments and coordinate comprehensive care plans.
• Collaborate with members, caregivers, physicians, and support networks.
• Monitor and adjust care plans for effectiveness and compliance.
• Promote integrated services including behavioral health and LTSS.
• Explain benefits and funding sources to members; document interventions.
• Lead ICT meetings and support team collaboration; use motivational interviewing.
• Identify barriers to care and assist with psychosocial and medical needs.
• Develop prevention plans for safety; maintain accurate electronic records.
• Travel 25–40% locally for member visits; mileage reimbursed.
