Job summary

Adobe Population Health (APH) is seeking a part-time Community Care Coordinator in Kingman, AZ. This role combines field-based care coordination with supervisory responsibilities to improve client outcomes through behavioral, physical, and social interventions.

The ideal candidate will travel within Mohave County, engage with diverse communities, and coordinate services to support health and wellbeing. A related degree or licensure, BLS, and a valid driver’s license are required.

Qualifications

• Four (4) years of related experience and/or training.
• Experience with diverse populations and underserved communities.
• Willingness to drive with a clean driving record and reliable transportation.
• Excellent verbal and written communication; problem solving and time management.
• Ability to work in a self-directed environment and adapt to changes.
• Knowledge of Quality/HEDIS/STARS measures.
• Proficient with Microsoft Office Suite; basic EMR literacy.

Responsibilities

• Provide field-based care coordination addressing behavioral, physical, and social needs.
• Assess social, financial, and health needs of individuals and families.
• Connect clients with housing, food, healthcare, transportation, and mental health resources.
• Develop individualized care plans with clients and follow up on progress.
• Advocate for clients to ensure timely access to services.
• Conduct home safety evaluations and telehealth coordination with NPs.
• Maintain EMR documentation compliant with HIPAA.
• Travel to Kingman/Mohave County for in-home visits and community events.
• Participate in outreach, education, and program evaluation.
