Job summary

Adobe Population Health is seeking a Part-Time Community Care Coordinator to provide field-based care coordination and in-home support in Eugene, Oregon. This role emphasizes collaboration with Nurse Practitioners for telehealth visits and a focus on equitable access to health resources.

The position requires travel around Lane County, strong communication, and experience working with diverse communities to connect clients with housing, healthcare, and social services.

Qualifications

• Four (4) years of related experience and/or training in health care or social services.
• Medical experience.
• Experience with diverse populations, including marginalized communities.
• Willingness to drive with a clean driving record and reliable transportation.
• Excellent communication skills, verbal and written.
• Ability to work in a self-directed environment and problem-solve.

Responsibilities

• Provide field-based care coordination to address behavioral, physical, and social needs of clients.
• Assess social, financial, and health needs within the community.
• Connect clients with housing, food security, healthcare, transportation, and mental health resources.
• Develop individualized care plans and conduct follow-ups and reassessments.
• Advocate to ensure timely access to services and resources.
• Facilitate telehealth visits with Nurse Practitioners and prepare vitals/documentation.
