Job summary

Adobe Population Health in Prescott, AZ is seeking a Community Care Coordinator (PT) to provide field-based care coordination and management for members in Yavapai County. The role blends in-home support with telehealth collaboration to address behavioral, physical, and social needs.

You will travel within the territory, develop individualized care plans with clients and families, connect to housing, food, healthcare and social services, and advocate to improve health outcomes in a part-time,

Qualifications

• Four years of related experience and/or training.
• Excellent communication skills, both verbal and written.
• Experience working with diverse populations, including underserved communities.
• Willingness to drive with a clean driving record.
• Reliable transportation.
• Proficient in Microsoft Office Suite.

Responsibilities

• Provide field-based care coordination to address behavioral, physical, and social needs.
• Assess social, financial, and health needs of individuals and families.
• Connect clients with housing, food security, healthcare, transportation, and mental health resources.
• Develop and implement individualized care plans with clients.
• Provide ongoing support, follow-ups, and reassessments.
• Advocate for clients to ensure timely access to services and resources.
