Job summary

Adobe Population Health seeks a part-time Community Care Coordinator (AZ - Prescott/Yavapai County) to provide field-based care coordination and in-home support for members. The role requires travel, strong communication, and a focus on reducing barriers to health and well-being.

The ideal candidate will have 4+ years of relevant experience, an associated degree or licensure, and familiarity with HIPAA and quality measures. This position offers part-time hours in Prescott, AZ.

Qualifications

• Four (4) years of related experience and/or training, or an equivalent combination of education and experience.
• Medical experience required.
• Experience with diverse populations, including marginalized and underserved communities.
• Must have a clean driving record and reliable transportation.
• Excellent verbal and written communication skills.
• Strong problem-solving and decision-making abilities.
• Ability to work independently in a self-directed environment.
• Knowledge of Quality/HEDIS/STARS measures and HIPAA compliance.
• Ownership of quality and timeliness of work; adaptable to changing needs.

Responsibilities

• Provide field-based care coordination to address behavioral, physical, and social needs.
• Travel within Prescott/Yavapai County for in-home visits and telehealth support.
• Coordinate with community resources to improve access to services and prevent gaps in care.
• Develop and monitor individualized care plans with clients and caregivers.
• Document interactions and outcomes in EMR, ensuring HIPAA compliance.
