Job summary

Adobe Population Health is seeking a part-time Community Care Coordinator to provide comprehensive care coordination in Prescott and surrounding Yavapai County. The role blends field work with client advocacy to improve health outcomes.

The position requires travel to member homes, collaboration with healthcare and social service partners, and strong communication skills. In-office and in-home visits may be involved as part of a mission-driven team.

Qualifications

• Associate’s degree in social work, sociology, psychology, counseling, or a related field (or equivalent).
• Current BLS certification and TB testing required.
• Current and valid driver’s license with willingness to drive.

Responsibilities

• Provide field-based care coordination to address behavioral, physical, and social needs across assigned clients.
• Assess social, financial, and health needs of individuals and families in the community.
• Link clients to housing, food, healthcare, transportation, and mental health resources.
• Develop individualized care plans in collaboration with clients and conduct follow-ups and reassessments.
• Advocate to ensure timely access to services and resources; coordinate telehealth and home visits as needed.
