Job summary

Adobe Population Health seeks a part-time Community Care Coordinator to deliver field-based care coordination and in-home support in Kingman/Mohave County. The role focuses on behavioral, physical, and social interventions to improve client outcomes and reduce costs.

You will collaborate with healthcare providers and community resources, assess needs, develop care plans, and advocate for timely access to services while traveling within the assigned territory.

Qualifications

• Four years of related experience and/or training.
• Medical experience and working with diverse populations.
• Willingness to drive with a clean driving record.
• Excellent communication skills, both verbal and written.
• Knowledge of HIPAA regulations and confidentiality practices.

Responsibilities

• Provide field-based care coordination to address behavioral, physical, and social needs.
• Assess social, financial, and health needs of individuals and families.
• Connect clients with housing, food, healthcare, transportation, and mental health resources.
• Develop and implement individualized care plans in collaboration with clients.
• Travel to Kingman and Mohave County for in-home support.
