Action Health Staffing & Home Care Services is seeking a Certified Nursing Assistant to provide in-home care in the Williamston area. Work dates span Monday–Wednesday–Friday 9:00–12:30 and Tuesday–Thursday 11:00–15:00, with patient-centered duties tailored to each home setting.

The role emphasizes direct patient care, hygiene assistance, mobility support, and adherence to the Plan of Care while maintaining patient confidentiality and reporting any changes promptly.

Qualifications

• Maintain certification as a CNA (DFS-related requirements may apply).
• Strong communication with clients and families.
• Ability to read, write, and follow care plans accurately.
• Ability to perform physical tasks such as lifting or assisting with transfers.
• Compassionate, patient-centered approach to home care.

Responsibilities

• Provide direct patient care and assistance as assigned.
• Assist with personal hygiene and daily living activities.
• Support transfers, repositioning, and ambulation.
• Follow Plan of Care and document changes in EVV time sheets.
• Provide companionship and participate in client-appropriate activities.
• Record and report changes in patient condition to supervising staff.
• Maintain confidentiality and uphold patient rights.
