Job summary

CareSource in Cambridge, MA seeks a Clinical Care Manager – RN to provide in-home and community-based care coordination for dually-eligible enrollees with complex medical, behavioral, and social needs. You will assess, plan, implement, and monitor care, coordinating with providers and community resources.

This role requires MA RN license (or LCSW/LMHC with related experience) and willingness to travel within MA.

Qualifications

• Associates of Science in nursing or Master’s degree in social work/mental health counseling with independent license.
• Experience in care coordination or case management with dual-eligible populations preferred.
• Knowledge of Medicare/Medicaid and community resources required.

Responsibilities

• Engage enrollees in homes and community settings to build complex care plans.
• Collaborate with ICT and healthcare providers to coordinate services.
• Assess needs with comprehensive tools and monitor plan effectiveness.
• Coordinate access to housing, transportation, and social services.
• Travel to member, provider, and community visits as required.
