Job summary

Humana Inc. is seeking a Case Manager for the Wisconsin Rapids area to provide comprehensive care coordination to frail elders and adults with disabilities within the Inclusa/Humana team.

The role involves in-home assessments, care planning, and coordination with Field Care Nurses to ensure services are delivered safely in the least restrictive environment. Applicants should have a Bachelor’s degree in human services or related field with 1+ year of experience (or 3+ years in a different field).

• Bachelor's degree in any field with 3+ years of relevant experience.

Responsibilities

• Assess members' health and safety needs and develop a comprehensive Member Care Plan.
• Coordinate with a Field Care Nurse to deliver services in the least restrictive environment.
• Travel to members' homes for assessments and monthly contact by phone.
• Arrange non-medical support services such as housing, employment, and community resources.
• Maintain documentation, including case notes and MCP updates.
• Ensure cost-effective service delivery and assess risk with members.
