Once onboarded, you'll work alongside the dedicated CarePartner team focused on finding you the right matches based on location, client needs, and your availability.

Qualifications

• Minimum 6 months of caregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent).
• Access to a smartphone capable of running the CareLinx app (required for scheduling, documentation, and timesheets).
• Ability to communicate in English (bilingual caregivers strongly encouraged to apply).
• Reliable transportation to and from client homes.
• Must pass a background check.
• Program-specific requirements may apply (training, TB test, medical clearance, or driver license/MVR check).

Responsibilities

• Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping
• Complete and document all care tasks accurately in the CareLinx mobile app during each shift
• Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team
• Submit timesheets on time through the CareLinx app
• Communicate with your CarePartner regarding any case updates or client changes
• Ensure patient privacy, dignity, and well-being in a safe and secure environment
• Maintain client confidentiality at all times
• Additional shift opportunities (based on client needs): Transportation and errand support
• Hospital discharge escort — safely accompanying members home after discharge
• Telehealth navigation — helping members connect with virtual doctor visits
