Job summary

CareLinx by Sharecare is hiring experienced caregivers to provide non-clinical, in-home care to health plan members and private clients in your area. You’ll work with a dedicated CarePartner team to find matches based on location and availability, and you’ll stay engaged via the CareLinx app to build consistent schedules.

Qualifications

• Minimum 6 months caregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)
• Access to a smartphone capable of running the CareLinx app (required for scheduling, documentation, and timesheets)
• Ability to communicate in English (bilingual caregivers strongly encouraged to apply)
• Reliable transportation to and from client homes
• Must pass a background check
• Completion of program-specific training prior to accepting shifts
• Medical clearance such as TB test, physical exam, or drug screening
• Valid driver's license and MVR check if accepting driving shifts

Responsibilities

• Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping
• Complete and document all care tasks accurately in the CareLinx mobile app during each shift
• Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team
• Submit timesheets on time through the CareLinx app
• Communicate with your CarePartner regarding any case updates or client changes
• Ensure patient privacy, dignity, and well-being in a safe and secure environment
• Maintain client confidentiality at all times
• Transportation and errand support
• Hospital discharge escort — safely accompanying members home after discharge
• Telehealth navigation — helping members connect with virtual doctor visits
