Job summary

CareLinxby Sharecare is seeking experienced in-home caregivers to provide non-clinical care to health plan members and private clients in your area. You’ll match with clients via the CareLinx app and control your availability, with part-time or per-diem shifts.

Onboarded caregivers work with the CarePartner team to find the right matches based on location and needs. This role focuses on compassionate, dignified care in clients’ homes and flexible scheduling.

Qualifications

• Minimum 6 months of caregiving experience or relevant CNA/HHA/MA certification.
• Access to a smartphone capable of running the CareLinx app.
• Ability to communicate in English.
• Reliable transportation to client homes.
• Must pass a background check.
• Program-specific training and TB/medical clearance may be required.

Responsibilities

• Assist clients with activities of daily living (ADLs) and daily living tasks.
• Document care tasks accurately in the CareLinx mobile app.
• Monitor changes in client condition and report to the CareLinx clinical team.
• Submit timesheets on time through the CareLinx app.
• Coordinate with CarePartners on case updates.
• Maintain patient privacy and confidentiality.
• Provide transportation and errands as needed.
• Support hospital discharge or telehealth navigation when required.
