Job summary

A First Name Basis (AFNB) is expanding its in-home care services across multiple states, seeking a Client Care Specialist to lead in-home assessments, caregiver training, and care-plan documentation that aligns with Medicaid and EVV requirements. The role is 80% field-based and 20% office-based, with competitive pay and comprehensive benefits.

Qualifications

• Strong interpersonal skills for in-home interaction and caregiver coaching.
• Willingness to travel locally for client visits and training.
• Detail-oriented in documentation and able to use tablet/app-based tools.
• Able to de-escalate and respond calmly to in-home incidents.

Responsibilities

• Conduct in-home assessments and client visits.
• Provide caregiver field training and onboarding.
• Develop and update individualized care plans based on home findings.
• Document visit notes to Medicaid-compliant standards.
• Maintain client files and documentation for audits.
• Respond to incidents and elevate as needed.
• Monitor caregiver performance during field visits.
• Ensure EVV/compliance readiness and accurate records.
