Job summary

A First Name Basis Home Care is seeking a Client Care Specialist to serve as the primary field-based contact for clients and caregivers. You will perform in-home assessments, provide hands-on caregiver training, and ensure that care plans and documentation reflect real conditions observed in the home, including field-level checks that keep Medicaid documentation and EVV compliance audit-ready.

This role is 80% field based and 20% office based.

Qualifications

• High school diploma or GED required.
• 1+ years of experience in home care, healthcare, or a related field.
• CNA or MA certification required.

Responsibilities

• Conduct in-home assessments.
• Client home visits.
• Caregiver field training.
• Administrative duties as needed.
• Conduct initial client intake assessments.
• Develop and update individualized care plans based on home assessment findings.
• Provide caregiver orientation and onboarding.
• Document visit notes and maintain client files per compliance standards.
• Respond to client/caregiver incidents and elevate as needed.
• Monitor caregiver performance during field visits.
• Document visit notes and assessment findings to satisfy Medicaid audit and payer documentation requirements.
