Job summary

A First Name Basis (AFNB) is expanding its team of in-home care professionals in the Denham Springs area. The Client Care Specialist is the primary field-based contact for clients and caregivers, conducting in-home assessments, delivering hands-on caregiver training, and ensuring that care plans and documentation reflect real conditions observed at each visit.

This role is 80% field-based and 20% office-based, requiring reliable transportation and strong attention to detail to maintain Medicaid

Qualifications

• 1+ years of experience in home care, healthcare, or a related field.
• CNA or MA certification required.
• High school diploma or GED required.
• Strong interpersonal skills for in-home client interaction and caregiver coaching.
• Comfortable traveling locally for home visits; reliable transportation required.
• Detail-oriented documentation habits, including comfort with tablet- or app-based assessment tools.
• Ability to de-escalate and respond calmly to in-home incidents.

Responsibilities

• Home assessments.
• Client home visits.
• Caregiver field training.
• Administrative duties as needed.
• Conduct initial client intake assessments.
• Develop and update individualized care plans based on home assessment findings.
• Provide caregiver orientation and onboarding.
• Document visit notes and maintain client files per compliance standards.
• Respond to client/caregiver incidents and elevate as needed.
• Monitor caregiver performance during field visits.
• Document visit notes and assessment findings to a standard that satisfies Medicaid audit and payer documentation requirements.
