Job summary

Monogram Health is seeking a Registered Nurse Care Manager to lead in-home management for patients with chronic kidney disease. You will partner with providers to adapt care plans, perform home visits, and monitor health data to drive better outcomes.

Responsibilities include patient education on CKD/ESRD, coordinating medications, and engaging families to support adherence. Local travel is required with occasional training travel to Brentwood, TN.

Qualifications

• Graduate of an accredited School of Nursing and active RN license in the posting state.
• Basic Life Support (BLS) certification required; employer supports certification on onboarding.
• 2+ years experience in care management and/or CKD/ESRD patient care.
• Ability to travel locally for in-home visits and occasional training travel.

Responsibilities

• Develop and continually adapt patient care plans with medical providers.
• Perform in-home care management visits and execute the care plan.
• Monitor biometric data and follow approved intervention protocols.
• Coordinate medications with pharmacists and prescribers; reconcile stock.
• Conduct health assessments and patient surveys as required.
• Provide education on CKD/ESRD, dialysis, and related comorbidities.
• Encourage adherence and engage family and social supports.
• Be the primary contact during business hours for patient questions.
• Educate on medications, conditions, diet, exercise, and lifestyle choices.
• Discuss proactive care decisions including Advance Care Plans and ESRD options.
• Obtain vital signs during visits and escalate concerns as needed.
• Engage patients from enrollment through onboarding and transitions.
