Job summary
Monogram Health is seeking a Care Manager – Registered Nurse to join our home-based care team. You will build trust with patients, develop and update personalized care plans, and lead in-home visits to support CKD/ESRD management.
The RN works with a small care team and coordinates with physicians, pharmacists, and dialysis providers to optimize outcomes. You will educate patients on medications, lifestyle, and proactive care decisions, while maintaining strong communication with families and
Qualifications
- RN licensed in the State of the posted location
- Bachelor/Graduate of an accredited School of Nursing
- 2+ years in care management and/or CKD/ESRD
- BLS certification required; onboarding support provided
Responsibilities
- Develop and continually adapt care plans with patients' medical providers
- Perform in-home care management visits
- Monitor biometric data and follow protocols
- Reconcile medications with pharmacists and prescribers
- Educate patients on CKD/ESRD, dialysis, and comorbidities
- Encourage adherence and engage family and support groups
- Coordinate with dialysis providers for seamless transitions
