Job summary

Monogram Health Inc. seeks a Care Manager RN to lead in-home care for patients with CKD/ESRD. You will partner with physicians to tailor and update care plans, perform home visits, monitor health data, and coordinate treatments to improve quality of life.

The role requires active RN licensure, at least 2 years in care management, and willingness to travel locally. You will educate patients and families and support continuity of care across providers.

Qualifications

• Graduate of an accredited School of Nursing.
• Currently licensed as a Registered Nurse in the state of the posted location.
• 2+ years experience in care management and/or with CKD/ESRD patients.
• Frequent local travel; some domestic travel may be required.
• BLS certification required; company supports certification after onboarding.

Responsibilities

• Develop and continually adapt care plans with patients’ medical providers.
• Perform in-home care management visits to execute care plans.
• Monitor biometric data and follow protocols for interventions.
• Inventory and reconcile medications; coordinate with pharmacists.
• Educate patients on CKD, ESRD, dialysis, and related conditions.
• Coordinate with dialysis providers to ensure seamless transitions of care.
