Job summary

Monogram Health is seeking a Care Manager Registered Nurse to partner with patients in their homes, coordinating care alongside a care team including an APP and a social worker. You will build trust, implement adaptive care plans, and monitor patient progress to improve quality of life and slow kidney disease progression.

The role involves in-home visits, close collaboration with physicians, and delivering education on medications, lifestyle, and proactive care decisions.

Qualifications

• Active RN license in the state of the posted location.
• 2+ years of care management and/or CKD/ESRD experience.
• BLS certification required; company supports renewal on onboarding.
• Willingness to travel locally for in-home visits.
• Strong communication and empathy with patients and physicians.

Responsibilities

• Develop and continually adapt care plans with patients' medical teams.
• Perform in-home care management visits to execute care plans.
• Monitor biometric data and intervene per approved protocols.
• Coordinate with pharmacists and prescribers; reconcile medications.
• Educate patients on CKD, ESRD, dialysis, and comorbidities.
• Engage families and support groups in patient care.
