Job summary

Monogram Health is seeking a Care Manager – Registered Nurse to work in patients' homes, coordinating care with physicians and a care team. The role emphasizes empathetic outreach and managing chronic conditions to improve quality of life.

The RN will conduct in-home visits, monitor biometric data, educate patients and families, and ensure smooth transitions between care settings. Occasional travel to support programs may occur.

Qualifications

• Graduate of an accredited School of Nursing.
• Currently licensed as a Registered Nurse in the state of the posted location.
• 2+ years experience in care management and/or CKD/ESRD patient care.
• BLS certification required; company supports certification on onboarding.

Responsibilities

• Develop and continually adapt care plans with patients and providers.
• Perform in-home care management visits.
• Monitor biometric data and intervene per protocols.
• Coordinate medications with pharmacists and prescribers.
• Educate patients on CKD/ESRD, dialysis, medications, diet and lifestyle.
• Support family and social networks in care planning.
• Act as primary contact during business hours for patients.
