Job summary

Monogram Health is seeking a Care Manager – Registered Nurse to lead in-home care management for patients with chronic conditions, primarily CKD/ESRD. The RN will collaborate with a care team including an APN and a social worker to improve quality of life.

Expect local travel for in-home visits, education, and coordination with physicians, pharmacists, and dialysis providers. Onboarding supports certification and ongoing professional development.

Qualifications

• Frequent local travel to perform in-home visits.
• BLS certification required; onboarding supports certification.
• Current RN license in the state of the posting location.
• Graduate of an accredited School of Nursing; RN licensure required.

Responsibilities

• Develop and continually adapt patient care plans with providers.
• Perform in-home care management visits to execute plans.
• Monitor biometric data and follow protocols for interventions.
• Inventory and reconcile medications; coordinate with pharmacists and prescribers.
• Educate patients on CKD, ESRD, dialysis, and comorbidities.
• Support adherence and engage families in care education.
• Provide education around medications, diet, exercise, and lifestyle choices.
• Coordinate with dialysis providers to ensure seamless transitions of care.
