Job summary

Monogram Health is seeking a motivated Registered Nurse to join as Care Manager. You will manage comprehensive in-home care for patients with chronic kidney disease and related conditions, coordinating with a multidisciplinary team to improve quality of life.

You will conduct in-home visits, monitor patients, educate families, and help navigate treatment decisions and adherence. Local travel is required, with some training travel as needed.

Qualifications

• Active RN license in the state of posted location.
• Graduate of an accredited School of Nursing.
• 2+ years experience in care management or CKD/ESRD care.

Responsibilities

• Develop and continually adapt care plans with patients and providers.
• Perform in-home care management visits.
• Monitor biometric data and follow care protocols.
• Coordinate medications with pharmacists and prescribers.
• Educate patients on CKD/ESRD, dialysis, medications, and lifestyle.
