Job summary

Monogram Health is seeking a skilled Registered Nurse to join our Care Team as a Care Manager RN. You will help patients live better at home and slow CKD progression by coordinating in-home care and collaborating with providers.

You will build trust with patients, perform visits, educate on treatment options, and ensure smooth transitions of care. Local travel and bilingual skills are preferred for this role.

Qualifications

• Graduate of an accredited School of Nursing.
• Currently licensed as an RN in the state of the posted location.
• 2+ years experience in care management and/or with CKD/ESRD patients.
• Willingness to travel locally for in-home visits.

Responsibilities

• Develop and adapt care plans with patients’ medical providers.
• Perform in-home care management visits.
• Monitor biometric data and follow care protocols.
• Reconcile medications with pharmacists and prescribers.
• Conduct patient health assessments and education on CKD/ESRD, dialysis, and comorbidities.
• Engage family and social supports in education and care.
