Job summary

Monogram Health is seeking a Registered Nurse to join the care management team to deliver in-home care to patients with chronic conditions. You will develop and continually adapt individualized care plans in collaboration with patients’ medical providers, aiming to improve quality of life and slow disease progression.

The role requires 2+ years in care management or CKD/ESRD, strong communication, empathy, and the ability to work independently with minimal supervision.

Qualifications

• Licensed RN in the state of the posted location.
• 2+ years of care management or CKD/ESRD experience.
• BLS certification required; company supports certification on onboarding.
• Willingness to travel for in-home visits.

Responsibilities

• Develop and continually adapt care plans with patients' medical providers.
• Perform in-home care management visits.
• Monitor biometric data and follow protocols for interventions.
• Coordinate with pharmacists and prescribers on medications.
• Educate patients on CKD/ESRD, dialysis, and comorbidities.
• Encourage adherence through regular contact with patients.
• Engage family and social support groups in education and care.
• Act as primary contact during business hours.
