McLeod Home Care is seeking a Care Coordinator to support patients through their home care journey. The role involves coordinating appointments, managing records, and facilitating communication among clients, caregivers, and staff to ensure seamless care delivery.

The ideal candidate will have strong organizational abilities, familiarity with medical terminology, and excellent communication skills to liaise with healthcare providers and families.

Qualifications

• Previous experience in a medical setting is preferred.
• Familiarity with medical terminology is highly desirable.
• Strong organizational skills with the ability to manage multiple tasks.
• Excellent communication skills to interact with patients and healthcare professionals.
• Proficiency in office software for documentation and scheduling.
• Medical administrative support background is a plus but not required.

Responsibilities

• Serve as the primary point of contact for clients, addressing inquiries and guidance on care processes.
• Build relationships with caregiver team.
• Schedule and coordinate caregiver schedules.
• Schedule and coordinate patient appointments with healthcare providers for timely access.
• Maintain accurate patient records, including histories and treatment plans, with confidentiality.
• Collaborate with medical and dental offices to streamline patient care and referrals.
• Assist with administrative tasks like insurance verification, billing inquiries, and follow-up communications.
• Educate clients about treatment options and available resources to understand care plans.
• Ensure compliance with regulations and standards in a medical or dental office setting.
