Job summary

Giving Home Health Care, LLC is seeking a Patient Coordinator to join our team, conducting home visits, attending physician appointments, and building strong relationships with patients to ensure their medical needs are met. You will manage a caseload of 50-60 patients, coordinate with nurses and aides, ensure proper DME, provide training, and maintain compliant documentation within 24 hours of visits.

Qualifications

• Associate degree preferred; high school diploma required.
• Experience in Case Management/Social Work preferred (not required).
• Knowledge of HIPAA and medical terminology is helpful.

Responsibilities

• Conduct home visits to assess patient needs and gather compliance documents.
• Ensure patients have proper DME and train patients, aides, and nurses.
• Conduct weekly/biweekly/monthly check-ins with nurses and home health aides.
• Manage caseload of 50-60 patients and coordinate with families and providers.
• Attend physician appointments to advocate for patients and explain the program.
• Update documents within 24 hours of visits and ensure compliance.
• Assist in patient assessments and care plan processing.
• Handle payroll assistance and contractor compliance tasks as needed.
• Report and document incident reports as required.
• Support other care team roles and cover night/weekend changes if needed.
• Meet and exceed performance metrics.
