Job summary

Humana Care Coach 1 in Florida focuses on assessing member needs during home visits and coordinating services to promote optimal wellbeing. You will develop member-specific care plans, collaborate with providers and families, and ensure access to appropriate resources while meeting Humana's care standards.

The role requires qualifications in health services fields, Florida licensure (RN/LPN) or case work experience, and willingness to travel locally.

Qualifications

• Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology).
• RN license in Florida with at least 2 years of relevant experience.
• LPN license in Florida with at least 4 years of relevant experience.
• Case Worker with a minimum of 6 years of Health Services/Case Management Experience.
• Prior experience with Medicare & Medicaid recipients.
• AHCA Fingerprint Required.
• TB screening is required for Humana's TB program.
• Valid Florida driver's license and personal vehicle liability insurance.
• Schedule Monday-Friday 8:30am-5:00pm.

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long Term Care Facilities – 75-90% local travel.
• Assess and evaluate member needs to establish a member-specific care plan.
• Ensure services are provided in the least restrictive setting to promote well-being.
• Plan and implement interventions to meet identified needs.
• Coordinate services and monitor the case management plan against member goals.
• Guide members/families toward resources appropriate for their care.
