Qualifications

• LPN license registered in Florida with relevant experience.
• Case Worker experience in health services/case management.

Responsibilities

• Visit Medicaid members in homes, ALFs, or LTC facilities with 75–90% local travel.
• Assess and evaluate member needs to establish a member-specific care plan.
• Ensure services are delivered in the least restrictive setting to promote well-being.
• Plan and implement interventions to meet member needs.
• Coordinate services and monitor the case management plan against goals.
• Guide members and families toward appropriate care resources.
• Collaborate with payer sources, providers, and interdisciplinary teams as required.
