Job summary

Humana seeks a Care Coach 1 in Florida to evaluate member needs, develop care plans, and connect them with services to promote wellbeing. The role involves meeting members in their homes or care facilities, with 75-90% local travel, and collaborating with interdisciplinary teams to drive health outcomes.

The position requires a Bachelor's Degree with 2+ years of relevant experience (or RN/LPN with experience). Normal hours are Monday–Friday 8:30am–5:00pm.

Qualifications

• Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology).
• Registered Nurse (RN) licensed in Florida with at least 2 years of relevant experience.
• Licensed Practical Nurse (LPN) licensed in Florida with at least 4 years of relevant experience.
• Case Worker with minimum 6 years of Health Services/Case Management experience.
• Must live in northern Pinellas county, Florida.
• Intermediate to advanced computer skills (Word, Excel, Outlook).

Responsibilities

• Visit Medicaid members in homes, Assisted Living Facilities and Long Term Care Facilities (75-90% local travel).
• Assess and evaluate members' needs to establish a member-specific care plan.
• Ensure services are provided in the least restrictive setting to promote wellbeing.
• Plan and implement interventions to meet member needs.
• Coordinate and monitor services; evaluate the care plan against member goals.
• Guide members and families toward appropriate care resources.
