Job summary

Humana is seeking a Care Coach 1 to visit Medicaid members in their homes and care settings, assess needs, and connect them with services to improve well-being.

The role emphasizes coordinated care planning, collaboration with interdisciplinary teams, and ensuring services align with members’ goals. Excellent communication and eligibility for Florida licensure where required are important.

Qualifications

• Bachelor's or nursing credentials with relevant healthcare case management experience.
• Florida licensure for RN or LPN where applicable and related work history.
• Experience with Medicare & Medicaid recipients is advantageous.
• Bilingual English/Spanish with ability to PASS language exams.
• Residence in listed FL areas and valid driver's license with insurance.

Responsibilities

• Visit Medicaid members in homes or care facilities with 75-90% local travel.
• Assess and develop member-specific care plans based on needs.
• Ensure services in least restrictive setting to support well-being.
• Plan and implement interventions to meet needs and goals.
• Coordinate services and monitor the case management plan.
• Guide members and families to appropriate care resources.
• Interact with payer sources, providers, and interdisciplinary teams as required.

Education

Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology)

Registered Nurse (RN) license in Florida with at least 2 years of relevant experience

Licensed Practical Nurse (LPN) license in Florida with at least 4 years of relevant experience

Case Worker with a minimum of 6 years of Health Services/Case Management Experience
