Job summary

Humana seeks a Care Coach 1 in Florida to visit Medicaid members in their homes, ALFs, and LTC facilities, assess needs and create member-specific care plans. You will coordinate services to promote wellbeing and meet member goals while traveling locally up to 90%.

Requirements include a Bachelor's degree with 2+ years of health services/related experience or Florida RN/LPN licensure, residency in listed FL areas, and ability to pass language exams in English/Spanish.

Qualifications

• Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology)
• RN or LPN license in Florida with relevant experience
• Case Worker with 6+ years in Health Services/Case Management
• Florida residency in listed areas
• Ability to pass Language Proficiency Exams (English/Spanish)

Responsibilities

• Visit Medicaid members in homes, Assisted Living Facilities, and Long Term Care Facilities; 75-90% local travel
• Assess and establish member-specific care plans
• Ensure services in least restrictive setting to promote wellbeing
• Plan and implement interventions to meet needs
• Coordinate services and monitor the case management plan
• Guide members and families to relevant care resources
• Engage with providers and interdisciplinary teams as required
