Job summary

Humana is seeking a Care Coach 1 to support Medicaid members in Florida by visiting homes and care settings, assessing needs, and building tailored care plans. You will partner with families to connect services that promote wellbeing and health outcomes.

You will work in member homes or facilities with local travel, coordinating with a multidisciplinary team and adhering to Humana's TB, background screening, and driver requirements to ensure safe, high-quality care.

Qualifications

• Bachelor's degree with at least 2 years of relevant health services experience.
• RN license in Florida with at least 2 years of relevant experience.
• LPN license in Florida with at least 4 years of relevant experience.
• Case Worker with a minimum of 6 years of Health Services/Case Management experience.

Responsibilities

• Visit Medicaid members in homes, Assisted Living Facilities, and other care settings – 75-90% local travel.
• Assesses and evaluates member's needs to establish a member-specific care plan.
• Coordinate services and monitor the care plan against member goals and needs.
• Guide members and families toward resources appropriate for their care.
