Qualifications

• Bachelor's degree with 2+ years of relevant health services experience.
• RN or LPN license in Florida with required experience.
• Case Worker with 6+ years in health services/case management.
• Pass language proficiency exams for bilingual role and reside in listed areas.

Responsibilities

• Visit Medicaid members in homes or facilities with 75-90% local travel.
• Assess needs and establish a member-specific care plan.
• Ensure services in least restrictive setting to promote wellbeing.
• Plan and implement interventions to meet needs.
• Coordinate and monitor the case management plan against goals.
• Guide members and families to appropriate resources.
• Interact with payer sources, providers, and interdisciplinary teams.
