Comfort Keepers #613 in Monroe Township, NJ is seeking a Client Care Manager to serve as the primary contact for clients and families, coordinating in-home care from assessment through ongoing services.

You will collaborate with caregivers and partners to deliver personalized care, ensure high-quality service, maintain documentation, and support agency growth through outstanding customer service and relationship building. This is an in-person, full-time role.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree preferred.
• Valid driver's license with reliable transportation.
• Excellent communication and interpersonal skills.
• Strong organizational and time management abilities.
• Proficiency with Microsoft Office.

Responsibilities

• Conduct in-home client assessments and develop individualized care plans.
• Coordinate the start of care and ensure services are delivered according to client needs.
• Perform routine supervisory visits and wellness check-ins.
• Maintain ongoing communication with clients, families, caregivers, and healthcare providers.
• Address client concerns promptly and ensure high levels of satisfaction.
• Collaborate with scheduling to ensure appropriate caregiver–client matches.
