The position includes performing comprehensive in-home assessments, creating care plans, and supervising Home Health Aides, with a focus on safety and client well-being.

Qualifications

• Active New Jersey RN license required.
• Minimum 2 years RN experience in acute care, home care, or case management.
• Experience with ventilator and tracheostomy care, G-tube/PEG, IV infusions, and wound care.

Responsibilities

• Conduct comprehensive in-home nursing assessments.
• Develop and oversee individualized care plans.
• Obtain and maintain physician orders per NJ regulations.
• Monitor client conditions and report significant changes.
• Complete accurate clinical documentation.
• Perform Home Health Aide supervisory visits and ensure quality of care.
• Provide clinical guidance to Home Health Aides and Nurses.
• Promote client safety, independence, comfort, and well-being.
