Job summary

Comfort Keepers in Monroe Township, NJ seeks an experienced Client Care Manager to serve as the primary contact for clients and families from assessment through ongoing care.

The ideal candidate will coordinate with caregivers and community partners to deliver high-quality, personalized care, while actively contributing to agency growth through excellent customer service and relationship building.

Qualifications

• High school diploma or equivalent.
• Bachelor's degree preferred.
• Valid driver's license with reliable transportation.
• Excellent communication and interpersonal skills.
• Strong organizational and time management abilities.
• Proficiency with Microsoft Office (Word, Excel, Outlook).
• Ability to work independently while collaborating with a team.

Responsibilities

• Conduct in-home client assessments and develop individualized care plans.
• Coordinate the start of care and ensure services are delivered according to client needs.
• Perform routine supervisory visits and wellness check-ins.
• Maintain ongoing communication with clients, families, caregivers, and healthcare providers.
• Address client concerns promptly and ensure high levels of satisfaction.
• Collaborate with the scheduling team to match caregivers with clients.
