Concierge Home Care in Monroe, NJ is seeking an experienced Registered Nurse (RN) Case Manager to provide in-home clinical oversight. This field-based role suits an organized RN who builds relationships with clients while delivering personalized, high-quality care in the home.

As an RN Case Manager, you will conduct in-home assessments, develop and oversee care plans, obtain physician orders, monitor changes, complete documentation, supervise Home Health Aides, and support the care team.

Qualifications

• Active New Jersey RN license.
• Minimum 2 years RN experience in acute care, home care, or case management.
• Current BLS Certification.
• Current physical examination within the past year.
• Current PPD/QuantiFERON or chest X-ray within past 5 years, if applicable.
• MMR immunization documentation.
• Valid driver's license and reliable transportation.
• Authorized to work in the United States.

Responsibilities

• Conduct in-home nursing assessments.
• Develop, implement, and oversee plans of care.
• Obtain and maintain physician orders per NJ regulations.
• Monitor client conditions and communicate changes.
• Complete accurate clinical documentation.
• Perform Home Health Aide supervisory visits and ensure quality.
• Provide clinical guidance and support to aides and nurses.
• Promote client safety, independence, comfort, and wellbeing.
